Patient registration.
Save 15 minutes at the front desk by filling out your registration forms online. Everything is secure, and your hygienist will have your medical history ready when she greets you.
Our patient registration link takes you to the secure Denticon portal — the practice management system we use. You’ll fill out demographics, medical history, dental history, and insurance details. It takes about ten minutes, and you can save and return if life interrupts.
If you’d rather fill the forms out in person, that’s fine too. Just plan to arrive about 15 minutes early so you have time to do them at the front desk before your appointment.
Bring with you to the visit: your insurance card, a photo ID, a list of medications you take, and any recent dental records or X-rays from a previous dentist (if you have them — we can also request them on your behalf).
